We will use this questionnaire to assist us in making a successful placement of a puppy or adult dog. It supplies us with information about your lifestyle, experience and goals as a potential Cardigan owner. Please fill out the questionnaire in its entirety with as much information as you can offer. At Daybreake Cardigans we believe we are responsable for this dog for its lifetime. Are you willing to share updates about the dog, as well as notifying us of any change of address. In addtion, should you no longer be able to keep this dog, we will require that we be notified of any changes. We will take return of ths dog at anytime, and will include in our contract that at no time is this dog to be given to a shelter or rescue.
